You're the only person who shared her interpretation before reading the meta! *hugs you* I looove your interpretation, which cleaves very close to mine. I also felt like "Sam" is sort of lost in his mind, at the mercy of his hallucinations/dreams, so the mind created Dean in an attempt to make himself feel safer and (as you said) give him a companion, someone to talk it all through with and go through it with, etc.
